yeah this is not a bug, its regional SMS limitation. Entra ID B2C uses Microsoft SMS providers behind the scenes and not all countries are fully supported for MFA SMS. Ghana is one of those “partial / unreliable” regions so u get
- messages not delivered
- numbers rejected
- inconsistent behavior
its not something u can fix in config tbh. if u are interested it happens coz
- telecom restrictions
- carrier filtering
- Microsoft providers not having stable routes there
I reccomend use something else for MFA, like
- authenticator app
- email OTP
- or custom SMS via external provider (Twilio etc via custom policies)
if u stay on built-in SMS u will keep getting random failures in those regions
